Clinical Practice Committee (CPC): Recent Activity
1. The Population Health team presented the March Population Health system scorecard. The system continues to do well in our preventative measures with opportunity in several of our chronic disease measures. The system improved 0.8% improvement in our HTN measure this month. Diabetes remained stable. Shoutout to Carris for improving in several measures this past month. Hannah Wieshalla, Director of QI and Network Support noted that we will receive our MNCM results back in May. This is when we review and look at benchmarks and goals we set for the next fiscal year.
2. Chelsie Bakken, RN, MBA, BSN, CPPS, Sr Director PE Improvement, presented inpatient measures:
- Carris all-cause readmission rate is trending slightly above goal at 10.3%.
- CentraCare Adverse Health Events: There were 6 adverse health events in quarter 2. Currently we do not have a benchmark or goal for this. This is part of the 2025 safety work.
- CentraCare C. Difficile Rate per 1000 Patient Days: Part of the composite scorecard which is their strategic KPI for this year.
- CentraCare Total inpatient Mortality Index: Above goal at 1.13%. Educating providers across the system around documentation that accurately shows the risk.
3. Sonya Wieber, MS, MBA, Sr. Director, Neurosciences, proposed a Neuroscience Practice Committee. The goal is to provide a venue for vetting new workflow processes and medication therapies to ensure safe and effective patient care. There was motion to sanction this committee.
4. George Morris, MD, Physician VP, Performance Excellence led the discussion around the role of CPC including our organizational restructure. CPC will continue to focus on Care Pathway for both HTN and Diabetes.
