Jane Monson, LPN, Care Coordinator

“As a care coordinator, I help people with transitions of care. I think about it as a blend of social work and nursing. You have to meet people on multiple levels, both for their physical and mental needs as well as what might be going on in their social environment. I work with a range of patients, including people who are homeless to a person who might just need a little help filling in the gaps for care. I’ve worked with babies as young as 6 months old to patients who are 95.

I grew up in Paynesville. I always knew I wanted to stay in a little town so I never had a desire to leave. Having grown up here, I know a lot of the people I care for. It really does make it easier to connect. There’s more of an immediate bond and trust. And trust is important because they may not always want to share what’s going on. One of the best feelings is when I meet with someone who may have a complex or challenging situation for a variety of reasons, and I’m able to peel through those layers and get things accomplished for them. That’s the best way to earn their trust. And once that happens, I can begin to help them and everything flows from there. I really do love my job.”
