Considering the time spent on clinical care may drive the level of service in some circumstances and environments. While Medical Decision Making usually determines level of service, very time intensive services can be represented by a higher level of service based on the time involved. The current definition is more in line with modern clinical practice — all of the time you spent in all tasks related to the care of a patient on the date of service counts (not just face-to-face efforts). NOTE: Billing on time does not apply to some environments, like Emergency Medicine outside of critical care in that setting.
